Qualifications
· 18 years or older and has a high school diploma or GED.
· At least 21 years old and is able to adequately communicate with recipients and staff/DSP/contractor providing direct care.
· Has a valid Social Security number or valid work permit if not a US citizen.

Job Duties
· Provide supports and assistance related to chosen outcomes on client's PCSP.
· Ensure supports are in compliance with Medicaid regulations.
· Collaborate effectively with each participant’s case manager and other support providers/agencies to ensure that services are person centered.
· Advocate for and be a resource to participants and their families. Allow self-determination and input from families to guide services.
· Ensure incident reports and follow up incident reports are filed and sent to case managers in a timely manner for all incidents concerning participants.
· Perform all other related duties, as required by the SCL and Michelle P. Medicaid waiver programs.
· Maintain cordial working relationships with outside agencies providing waiver services/case management.
· Maintain documentation in accordance with services provided.
Qualifications
· Must meet the requirements of a Direct Support Professional
· Previously qualified or credentialed by the department to provide community access services prior to the date of the updated regulations.
· At least one year of experience in the field of intellectual or developmental disabilities.
· Completed a department approved training program within one year of application while providing community access services under the direct supervision of a community access specialist
Job Duties
· Services shall be designed to support a participant to participate in meaningful routines, events, and activities through various community organizations.
· Shall be designed to empower a participant in developing natural supports
· Shall stress training that empowers a participant in acquiring, practicing, utilizing and improving skills related to: Connecting with others, independent functioning, self-advocacy, socialization, community participation, personal responsibility, financial responsibility, other skills related to optimal well being as defined by the participants person centered service plan.
· Shall assist the participant to increase ability to develop natural supports and access community resources including education, recreational, religious, civic or volunteer opportunities with an outcome of less reliance on formal supports and greater reliance on natural or unpaid supports as established in the person centered service plan.
· Shall assist the participant in development of personal social networks, membership opportunities, friendships, and relationships for the participant as established in the participant’s person centered service plan.
· Shall provide services outside the participants home or residential setting and occur during the day, evening or on weekends.
· Shall provide 1:1 services, staff to participant ratio or no more than two participants according to the person- centered service plan, if the participant invites a friend
· Shall provide services in an integrated community setting.
· Maintain documentation in accordance with services provided.
Qualifications
· Must meet requirements of Direct Support Professional providing direct care.
· Has at least one year of experience in the field of intellectual or developmental disabilities.
· Complete the HDI (KY SETP) Supported Employment training as soon as possible (the next time that the training is available), but it must be completed within 6 months of providing SCL/Michelle P Supported Employment services.
Job Description
· Ensure that the person centered job selection shall include a meeting at which parties supporting the participant provide suggestions of places in the participants area where the participant might be able to perform the job tasks identified in the person centered employment plan in return for at least minimum wage.
· Assist client in gathering contacts for such places of employment and ensure that this information gathered at the job planning meeting shall be documented in the participants individual plan for employment.
· Initiate and coordinate services through the office of Vocational Rehabilitation.
· Ensure that the job development and analysis for the participant be conducted to determine the skills that the participant will need to successfully contribute in specific workplace, include discussions of talking to the participant about the impact of the disability in relation to the contributions that the participant can offer the employer, include facilitating the development of natural supports based on social relationships at work.
· Supported Employment Specialist shall include matching job tasks that need to be completed for potential employers with the interest, skills and abilities established in the participant’s person-centered employment plan.
· Ensure that stabilization services shall include becoming as independent as is possible in the workplace through assistance from natural supports.
· Prior to initiating a long term support and follow up services, the employment specialist shall review the supported employment long-term plan with the participant and participant’s team to ensure that the participant is functioning well in the job in terms of general satisfaction, number of hours worked and performance of job duties, participant is comfortable in interacting with coworkers and supervisors and performs job duties through the use of natural supports and that the long- term support plan has been completed and integrated in the participants person centered service plan.
· Ensure that the long- term support and follow up plan shall be provided to help a participant maintain the job and experience continued success after the participant is fully integrated into the workplace and when the employment specialist is no longer needed on a regular basis.
· Ensure that if a participant needs an increase in supported employment services shall be justified in the long-term employment support plan and approved by the participant and participant’s person-centered team.
· Maintain documentation in accordance with services provided.

· At least two years of experience in the field of intellectual or developmental disabilities, OR 12 hours of college coursework in a human services field.
Job Description
· Ensure that all behavior support plans are implemented, as written under direction of the Behavior Support Specialist, or other licensed professional.
· Implement, train and monitor behavior support plan using positive behavior supports.
· Assists a participant and the participant’s person-centered team in implementing and monitoring the effectiveness of the participant’s person-centered service plan.
· Assists in training a participant, family, guardian, natural and unpaid supports, and other members of the person-centered team when barriers challenge the success of the participant in achieving his or her goals.
· Attend POC meetings, quarterly staff meetings, and psychiatric appointments when needed.
· Maintain constant communication with Behavior Specialist or other licensed professional, Case Manager, and other members of the person-centered team
